12 HUMAN QUALITY ERROR DETECTION AND PREVENTION CASES :
1. WINDOWS TO OPEN AUTOMATICALLY UPON SMOKE DETECTION:
Problem: There are various instances when houses explode due to short-circuit or due to gas leakage. During heavy storms especially, there are chances of houses catching fire due to short circuit which can lead to a massive fire in the entire neighborhood.
This is a serious problem, and it is caused due to human error. The gas should be put OFF when not in use; similarly, the electric circuits must be well taken care of and placed keeping in mind the safety and security. I have personally witnessed an incident where a family was stuck inside a fire engulfed apartment until help arrived because the fire was blocking the windows in the vicinity. This could have been proved very dangerous hadn't help arrived o time.
Detection: I feel an automatic smoke detector should be installed in every house and workplace which will be connected to all the windows and that can control their opening and closing as per requirement.
Whenever smoke is detected, the windows in the vicinity must open first and the ones far should open after that, automatically. This will prove to be very beneficial, as there will be no need of opening the windows manually in such critical circumstances.
Even the fire exit windows must open at the earliest, to facilitate exiting the place in the least time duration.
Prevention: Opening of the windows will also make this disastrous event slow down a little as the explosion will be more spread out, decreasing its intensity. This technology will prove to be very helpful and advantageous during natural calamities such as hurricane, storm, floods, etc.
This technology will help save many precious lives, houses and other assets of people.
I hope to see such a technology implemented in the near future.
2. BROKEN CHAIR:
Problem: Having a broken chair is one of the most common problems tha we come across in our day-to-day life.
Detection: It is not very practical to detect a broken chair without sitting on it, but we still can identify a broken chair by looking at it or inspecting it before sitting. If it looks out of shape or bent, then it is better to inspect it before sitting. In the above picture, one of the legs of the chair was broken. One of my roomates pointed it being out of shape luckily avoiding any damage.
Prevention: Although, this problem can't be entirely prevented, I feel it is better to have some kind of backup mechanism with the help of a rod or something just to make sure the seat does not fall off. The chair should be built with strong wood to ensure non-fragility and it should also be held together strongly at the edges.
3. DAMAGING SOMEONE ELSE’S CAR WHILE PARKING YOURS:
Problem: Damaging someone’s car while parking our own car. This is from my own experience when I had damaged another car while parking my own car on the street.
Detection: As I was parking my car on the street, there was already one car parked right in front and behind where i was supposed to park. I was parking my Hyundai Santro and due to less space between two cars it was difficult for me to park my car. As I was parking the car I banged the car behind and damaged its tailamps. I detected that this was a flaw and cost me a lot in repairing it. I was thinking if I had some way that I could avoid it and save on my pocket expense. This is not the case when u have a high tech car which has a rear camera and a sensor which sounds when u are closer to some obstacle this definitely avoids accidents. But people refrain from buying such cars due to the difference in the cost. It almost costs 30 to 40% more than its normal price.
Prevention: Bringing in the same technology for every car with a cutoff in the price and should be made compulsory which can avoid these types of accidents. Keeping in mind that if there is a mass production of a particular product the estimated cost for the production can also be less than usual one and can be affordable for people to buy a high tech car.
4. IMPROPER COLLECTION OF GARBAGE:
Problem: People throw garbage anywhere while walking off the road. This isn’t mostly seen in The United States but still seen a lot in India and many other countries. This leads to various problems like diseases and a spread of foul smell throughout that area. The above picture depicts the garbage scene outside my house everyday.
Detection: I have seen many people who just throw garbage outside the garbage bin in India and United States. This is very much prominent in India due to Population explosion but very well controlled in US due to low population and proper management skills. I have seen it myself when people go to throw garbage in the dump zone they just do not walk inside it they throw it from outside due to which 60% of the garbage is dumped outside the bin and due to this everyone dump their trash outside and this causes the development of various harmful insects which transmit disease in the nearby environment. I have been a target of such insects when I was attacked by dengue due to uncleanliness in the area nearby. This is the main reason which causes epidemics throughout a particular area in which many people are affected and even some of them lose their lives due to this.
Prevention: This can be prevented by following the same type of management which is used in the states and is a very good way of avoiding spread of diseases and maintaining a healthy environment. If there are bigger dump zones and are cleaned frequently they can maintain a healthy environment in which people can reside.
5. WASTAGE OF ELECTRICITY:
Problem: In our day to day life many of us forget to switch of the Lights and fans which are not being used at that particular time which causes the wastage of Electricity.
Detection At the evening when I was leaving my room after switching OFF all the switches I figured out that the Light of my closet was ON. This causes the lot of electricity wastages as nobody is using at that time. This is where the right approach isn't used to switch OFF the unwanted switches of lights and fans. By doing this there will be a lot of saving of electricity which can be used further.
Prevention: Immediately after leaving the place and seeing that there isn't anybody to use it further then switch off the useless switches so that forgetting of the same isn't the issue. Prevention can be taken by using the Occupancy sensors which switches off the Lights and fans when there is no one in the room.
6. ACCIDENTS:
Problem: Accidents occurring on the turns on motorcycles.
Detection: This was an accident occurred when I was at my place in india and it happened with one of my friends. He was heading home and was hit by another motorcycle on a turn and later it was found it was his fault because he did not turn his indicator on and both of them dashed eachother . In this I asked him dint he know that he has to turn on his indicator while he turns he replied to me that I thought that I have turned on the indicator but it did not turn on. Here I found a quality error .
Prevention: This could have been avoided if there would have been a beep sound for the indicator when it is turned on. It is not always possible to see whether your indicator has turned on or no. its better to have a beep when the indicator is wrong means it helps avoiding accidents.
7. GPS DEVICE ERRORS:
Problem: GPS should have better connection to the satellite signals. There should be more accurate tests performed because no matter how expensive a GPS device is, when it comes to getting where you want to go, it's really only as good as the satellite network and its map data. The quality to every aspect of this GPS including the system (hardware and software) is the main component. The second strike is that maps quickly go out of date. The directions your GPS device compiles are based on digital maps provided by a mapping and navigation company that has partnered with the device manufacturer. They should have a feature where you can update often because GPS should last for a long time and how would we, as the customers, know when roads, highways, exits, etc get changed?
Detection: There are techniques to improve accuracy. The accuracy of GPS can be improved several ways. For example, Differential GPS (DGPS) can improve the normal GPS accuracy of 4-20 meters (13-65 ft) to 1-3 meters (3-10 ft). DGPS uses a network of stationary GPS receivers to calculate the difference between their actual known position and the position as calculated by their received GPS signal. Relative Kinematic Positioning (RKP) is another approach for a precise GPS-based positioning system. In this approach, accurate determination of range signal can be resolved to an accuracy of less than 10 centimeters (4 in). This is done by resolving the number of cycles in which the signal is transmitted and received by the receiver.
Prevention: This can be accomplished by using a combination of differential GPS (DGPS) correction data, transmitting GPS signal phase information and ambiguity resolution techniques via statistical tests—possibly with processing in real-time (real-time kinematic positioning, RTK) and many automobiles that use the GPS combine the GPS unit with a gyroscope and speedometer pickup, allowing the computer to maintain a continuous navigation solution by dead reckoning when buildings, terrain, or tunnels block the satellite signals. This is similar in principle to the combination of GPS and inertial navigation used in ships and aircraft, but less accurate and less expensive because it only fills in for short periods.
8. FORGETTING YOUR HOUSE KEYS:
Problem: This has happened to me several times, I usually lock the door from inside when I'm leaving the house and sometimes I forget to take my keys or take a wrong set of keys. I am sure this happens to a lot people around the world. We all make this error from time to time. And unless we have a spare key anywhere outside the house like in a car or at a friend's place, we land in trouble.
Detection: Detection of this error would occur when we are unable to enter or access the area or device the key is supposed to open.
Prevention: One way to prevent this error would be to change the unlocking mechanism to reduce the number of keys needed. In automobiles, only one key is needed to open the side doors, the trunk and to start the engine. Another way to deal with this error would be to eliminate keys entirely. The use of biometrics is gaining popularity because it serves to identify individuals and grant them access to systems without the need of multiple passwords. In the future we will only need to scan our retina or our fingerprint to gain access to our car or our home.
9. CAR SEAT BELT WARNING SOUND/LIGHT:
Problem: The purpose of the seat belt in cars and other moving vehicles is to prevent users from injuring themselves in case of an accident.
Detection: In order to enforce the use of seatbelts, car manufacturers have installed sensors that detects whether the driver is using the seatbelt when the car is started. When the vehicle is started and the sensor is not engaged, the vehicle will either emit a beeping noise or turn on a light on the dashboard, or both. The beeping sound is usually at a volume that will make the driver uncomfortable, and will for the driver to comply by fastening the seatbelt. For some drivers, it is uncomfortable to wear a seatbelt while operating a vehicle. In other situations where seatbelts are present, like inside airplanes, or on roller coasters, there are other people that make sure passengers comply with this rule. But when operating vehicles alone, drivers are left to decide if they want to use seatbelts or not. There are ways that drivers have circumvented this process. For example, a driver can click the seatbelt to the plug without wrapping the seatbelt around them. Another way would be to disable the fuses that power the light/sound. And another way is for a driver to adapt to the beeping sound.
Prevention: While this probably would not be a popular alternative, sensors could be used to prevent an engine from starting unless the seatbelt is plugged and wrapped around a body. Vehicular Violation,
10. FAILURE TO STOP AT A STOP SIGN:
Problem: There is an intersection near my apartment where drivers regularly fail to make a stop before continuing down the street. Failing to stop at a stop sign is probably one of the most common violations in our transportation system. Stop signs are placed at intersections to force vehicle operators to reduce and/or stop and allow other vehicles to pass.
Detection: However, at my intersection this rule is frequently broken. Failing to stop at a stop sign could be classified as an unintended act, one caused by being distracted and not seeing the stop sign. It could also be classified as an intended action, as a driver who is in a hurry and knowingly fail to make a stop. I used to think drivers were being careless and that only them were at fault. But looking at the quality aspects of this process, there are some that could be improved as the current process lacks reliability and safety. At least in this case, it seems a stop sign is not enough to alert drivers to reduce the speed and stop.
Prevention: An intermittent signal would be more helpful. It would also be helpful if the stop sign were made bigger or more noticeable. Many towns are now installing blinking red lights around stop signs and powering them with mini solar panels. Also, the angles at this intersection are not quite aligned to make obvious the presence of the stop sign. Perhaps changing the layout of the street or the sidewalk to narrow this intersection would increase the awareness of the driver.
11. BLIND SPOT:
Problem : When I first heard what a blind spot is when driving, I did not take it seriously because I did not know exactly what it meant and how important it is when driving to know what it is.
Detection: Last year, I was involved in a vehicle accident where there was a guy who was switching lanes and I was driving in the lane next to him, right there. He hit me but when I saw that he was about to hit me, well I moved my car and an accident happened. We both pulled to the side and he told me he was not aware of his blind spot. Both were caused by non-mechanical issues. In other words, human error. The accident happened due to him switching lanes without a signal or knowing his blind spot. Unfortunately and we all know that vehicles have blind spots where the driver cannot see unless they change positions and orientation of their head to view certain points outside and around their vehicle. They move their head forward while checking the side mirror (driver side) for any vehicles in the left lane. If you just sit back and drive without moving the orientation of your head position and checking the side mirror, well, there is a high possibility you will be involved in an accident because of carelessness. Moreover, I have always made it a point when driving to have my blinker on for several seconds prior to changing lanes. The human detection know your blind spot of your vehicle to avoid in getting into any accident.
Prevention: Awareness of the blind sport is very critical when driving especially when you want to switch lanes. Switching lanes without being aware that there is vehicle right there next to you, well you have seen from my experience that this error caused an accident and most importantly, could have caused serious injuries. Accidents can happen very easily especially if we are not paying attention and focusing on the road and our surroundings. That is what the blind spot will tell you. That we need to focus and be aware of our surroundings! The cost, damage and serious injury are the factors to take into consideration when getting into a car accident. All of these things are tremendous. Moreover, for safety purposes, know your blind spot and car signals. This is the way to prevent accidents from happening in relation to switching lanes. Sometimes, humans refuse to take it seriously and then there are consequences. Like the guy that hit me because he didn’t know I was there. The failure to understand how serious blind spot can be and not paying attention to them when switching lanes is human error. It is only beneficial to be wise when driving because the well being of the person is at risk and so is the car. To prevent future accidents, we have to pay attention to these little things on the freeway and not take carelessness as a joke. Yes it could happen to anybody but things we can improve on will impact our future.
12. CONDITION OF A CAR:
Problem: Driving with a tank’s car open or with inadequate and worn tires when road is snow covered are the common mistakes that humans daily make.
Detection: People tend to deliberately ignore the signals displayed by the car’s board (a car’s tank once open makes an engine fault to light up on a car’s board), car drifting on the road covered by snow. Since the car can move, they don’t care. But all those are very dangerous because a simple ignition coming from everywhere or a drifting car can cause the irreparable.
Prevention: Automobilists must always adapt the tires of their car to the actual season. Depending on how many miles they drive per day, they should plan the replacement of the tires. Also, it is a hazard that a car’s board is displaying a faulty part. Always check or bring your car to the mechanic once you observe such event.
